What is a HC5 form?

The H5C form for dental care allows you to claim for NHS dental treatment refund.

To be eligible for a refund, you must absolutely het your dental care in England.

How to fill in the HC5 Dental Form?

Each person who paid for dental charges need to fill in a separate HC5 claim form.

To get a full refund, you will need to complete the above official HC5(D) form and include all receipts of dental charges you had to pay.

If you are filling in the form for someone who can't do so, specific Part 5A and Part 5B of the form will guide you through how to do it.

When to fill in the HC5 Dental Form?

You need to make a claim to get a refund within 3 months after you paid dental charges. If you ask for a refund after 3 months, you will need to write a letter to the NHS Business Service Authority explaining why you are claiming for a refund so late etc.
